Fife Supported Living Service is registered as a combined service of Housing Support service and Care at Home service. The service is managed by Tracy Young (Service Manager) and two Team Leaders, Joanne Clayton and Pamela Young. The service is provided to people in their own homes, and out in the community.
The service is available to adults with disabilities from age 16 years, living in Fife. The intensity of the service varies between individuals, some receiving 24 hours support, others having several visits of support over seven days a week (packages normally 30 hrs plus per week). People are supported to live as independently as possible in the community. The service has successfully supported people with complex and significant needs for several years, including support to exercise choice and control in their lives.
The service can provide support with the following:

Person Centred Planning Reviews and Assessment;

Personal Care Medication support;

Helping with Finances;

Complex Health Needs;

Living well and Wellbeing;

Any support required to ensure a high standard of daily life is experienced.
